Referrals can be submitted by phone, fax, or email. To refer a patient, we need: a physician order for physical, occupational, and/or speech therapy and documentation of a recent face-to-face visit with the referring provider. Our team will promptly coordinate with the patient, family, and care team to schedule an evaluation and begin services.
